This position offers the unique opportunity to work within a supportive team environment while operating directly in the homes of patients facing chronic illnesses, recovering from surgeries, or requiring long-term care. Our LVNs play a crucial role in the recovery and well-being of our patients, ensuring the highest quality of life while in the comfort of their homes. Provide high-quality patient care under the supervision of a registered nurse (RN). Administer medications, track vitals, and report patient status to supervising RN or doctor. Assist with daily living activities, including bathing, dressing, and grooming. Educate patients and their families on disease processes, medical procedures, and other relevant healthcare topics. Maintain accurate records of patient care, condition, progress, and concerns. Respond rapidly and effectively to emergency situations. Collaborate with multidisciplinary teams to improve patient care. Strong clinical skills in a healthcare setting Effective communication and interpersonal skills Proficiency in electronic medical record (EMR) systems Ability to work independently and as part of a team Compassionate and patient-focused approach Medical Insurance Dental Insurance Vision Insurance Life Insurance Current LVN license in the state of practice CPR certified Valid driver's license and reliable transportation Proven ability to handle confidential information with discretion
