WellMed’s proactive approach focuses on prevention and the complete coordination of care for patients. WellMed is now part of the Optum division under the greater UnitedHealth Group umbrella. Provides skilled nursing services that reflect Homecare Dimensions, Inc. philosophy and policies to assigned patients under the direction of the registered nurse and according to physician orders. Reflects Wellmed Medical Management philosophy and policies. Performs a variety of healthcare screening activities to assist providers in conducting quality clinics, health fairs, home visits, including but not limited to administering injections, diagnostic testing, phlebotomy, quality surveys, and various other procedures.  Delivers exceptional customer service and maintains established quality control standards.  This position works directly with our quality mobile team, contracted physician groups. This position is a major part to the  “ Mobile / Hybrid Team” This position requires 100% traveling around the San Antonio, TX and counties areas supporting Homecare Dimensions’ Patients. Must have reliable transportation and available to work flexible shifts. The main office is located near Network Blvd., 78249 Primary Responsibilities: Performs all duties and procedures within the scope of a Licensed Vocational Nurse (procedures, injections, diagnostic testing, phlebotomy, etc). Coordinates healthcare screening results as directed by providers, company standards, and policies. Travels with a mobile team to clinics and centralized locations to conduct screening clinics to support contracted physician network. Arrives to all clinics or designated locations at least 15 minutes early for introduction,  set up equipment in designated clinics or work areas. Organizes onsite work areas to ensure all equipment and supplies are on hand and set up in the exam rooms prior to receiving patients for screening appointments. Performs and completes all quality program health screenings per required metrics. Processes appropriate documents in an organized and accurate manner and copies all physicians on patient results and quality forms submitted. Reviews all patient screen forms to ensure all outstanding healthcare screenings are performed and reviews medical records to data mine and collect referral information for various screens: colon screenings, DM, eye exams, mammograms, labs, etc. Records patient care documentation in the medical records accurately, timely, and according to company policy. Conducts post audits on all information submitted to quality program to make sure information is complete and presented within the guidelines.  Any items rejected due to noncompliance will be researched, recorded, and resubmitted according to guidelines. Reviews all medical information, test results, and survey questions prior to the patient leaving to ensure all information is captured. Reports and presents abnormal results to the provider promptly to ensure there is not an emergent condition that needs immediate attention. Pre and Post work assignment preparation of mobile unit for faxing, filing, and forwarding to the appropriate staff. Works as a liaison between mobile team, physician business manager, and contracted MD offices to ensure all patient information is reported accurately and are completed.  Offers solutions to improve quality goals. Maintains confidentiality of all materials handled within the department as well as proper release of patient information at all times. Maintains quality control standards. Compiles results and enters information into utilization reports for submission to management Works collaboratively with mobile team staff, medical director, PBMs, and proiver offices to ensure completion of quality forms and improve overall quality score. Assists the registered nurse in carrying out the plan of care. Performs complete assessment of the patient's condition during every home health patient encounter. Assists the physician and/or home health care RNs in performing specialized procedures following the physician’s plan of care to include assessments, medication administration, wound care, therapeutic treatments, and skilled teaching as ordered and indicated. Prepares equipment and material for patient treatments, observing aseptic techniques as required. Assists patients in learning appropriate self-care techniques. Recognizes and reports life-threatening situations and responds appropriately. Reports changes in the patient's condition to the CTM on a daily basis. Recommends additional needed home health care services. Communicates to the patient's physician and documents verbal orders in a timely manner. Documents patient interventions that reflect delivery of safe and high-quality home health care on a skilled nursing note written during the patient encounter. Turns in all documentation within 48 hours of the visit or shift (visits/shifts performed on Thursday, Friday or Saturday are due on Monday). Documents all communications with the patient, family, physician, CTM, pharmacy, other disciplines, and appropriate others as indicated on communication notes. Attends case conferences to promote coordination of care. Implements infection control and safety measures per agency policy and procedure. Demonstrates effective written and verbal communication. Gives accurate information to patient and patient families regarding costs for home health care services.  Provides information regarding equipment and supply needs to the CTM in a timely manner. Complies with all agency policies and procedures.  Promotes and maintains agency environment that is in compliance with federal, state, and local regulatory guidelines. Participates in personal professional growth and development. Demonstrates flexibility, enthusiasm and willingness to cooperate while working with others or in place of others as necessary. Accepts other duties as assigned Required Qualifications: High school graduate or GED equivalent Graduate of an accredited school of nursing Current unrestricted licensure as a Vocational Nurse by the Board of Licensed Vocational Nurse Examiners for the State of Texas Maintains certification in Basic Cardiac Life Support Course C 1+ years of related LVN experience in a clinical setting or equivalent combination of education and experience preferred Basic computer literacy Knowledge of medical terminology Ability to define problems, collect data, establish facts, and draw valid conclusions Ability to interpret an extensive variety of technical, medical, and regulatory instruction and deal with numerous contemporaneous issues and problems involving delivery of patient care Ability to travel frequently within assigned market Ability to work flexible schedule to meet patients' needs; some exposures to inclement weather; rotating call and emergency call Must be able to read and write in English and effectively follow written and verbal instructions in English This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease Preferred Qualifications: 3+ years LVN experience in a clinical setting Successful demonstration as a LVN in physician offiee with  skills to teach, lead, and educate staff; understand the compliance of all quality procedures and clinical guidelines of the company clinic policies.
