Registered nurse license, Bachelor of Science in
NursingCertifications: Good standing as a licensed Registered Nurse
with the State of Texas Board of Nursing; current BLS
certifications (PALS preferred), First aid, CPR, Emergency behavior
interventionWork experience: Required - At least 5 years of
experience as a RN; Preferred - pediatric nursing
experienceCritical Action Items & Measurable Deliverables:
- Meet and maintain all federal and state regulatory guidelines
and standards applicable to this position.
- Provide telehealth triage services to assess patient needs and
evaluate symptoms. This includes answering patient inquiries,
providing patient teaching, and determining the urgency and
appropriate course of action.
- Conduct in-person and virtual assessments based on established
protocols and accurately document medical findings.
- Gather, collect, and clarify patients' current condition and
past medical history.
- Effectively communicate with Medical Director as needed for
reporting findings which are concerning for patient's health and
well-being.
- Communicate with RN Managers and other medical staff to devise
individual plans of care and acts as a nurse support to other
medical staff.
- Implement Compass Connections safety protocols including
evacuating with children and other staff in case of an
emergency.
- Maintain confidentiality of health care records and information
in keeping with HIPAA and Compass Connections confidentiality and
Personally Identifiable Information (PII) protocol and
procedures.
- Address other job duties as assigned or for unforeseen matters
that require attention to maintain a fluid and fully functional
program.Other Responsibilities:
- Plan, organize and, prioritize daily work using sound
judgement.
- Monitor established health care plans for service population at
the sites within the timeframe established by our State and Federal
partners.
- Ensure accurate documentation in accordance with agency
policies and procedures, regulatory requirements and standards, and
program timeline requirements.
- Meet all program and federal partners' documentation and
processes requirements.
- Other duties as assigned.Requirements:
- Pass a pre-employment drug screen and random drug screens
throughout employment.
- Provide proof of work eligibility status upon request.
- Pass a pre-employment and biennial criminal background
checks.
- Demonstrate the ability to:
- Work collaboratively with other staff members, service
providers and professionals.
- Communicate effectively in writing and verbally in
English.
- Work in a fast-paced environment and make decisions based on
the needs of the program and service population, while maintaining
emotional control and professional composure at all times.
- Maintain computer literacy required to meet the
responsibilities of the position.
- Work effectively and without intensive supervision both
independently and as a member of a multidisciplinary team.
- Demonstrate a working knowledge of all Compass Connections
policies and procedures.
- Must provide current physical-medical release and provide proof
of immunizations, titers, or declinations for the following:
- MMR
- Varicella
- Hep B
- Hep A
- Tetanus
- PPD
- Meningococcal (preferred)
- Must be able to perform physical activity such as extensive
walking, stretching, and bending.
- Must be able to work in a team environment.
- Must be able to maintain professional people skills, including
oral, written, listening, and non-verbal communication
