We can't wait to welcome you to the Strive Team!_*$2000 Bonus Eligible Position for Full-Time*_*JOB DUTIES** Assess home health pediatric patient for rehabilitative and functional status.* Regularly re-evaluate the patients/clients physical therapy need and evaluate the outcomes of care on a regular basis.* Physical Therapist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.* Identifies deficits including but not limited to muscle strength / coordination, mobility, gait, ROM and potential for intervention. Evaluates the home environment.* Physical Therapist evaluates the need for and recommends DME devices and aides.* Uses infection control measures that protect both the staff and the patient (OSHA).* Coordinates services.
